CARDIOLOGY CONSULTATION
Diagnosis:
1. Mitral Valve Disease Status Post Mitral Valve Replacement with a St. Jude
Prosthetic Valve and Coronary Artery Bypass x 1
2. History of 2 previous Cerebrovascular Accident secondary to an embolic
disorder from inadequate anticoagulation
3. Atrial Fibrillation come on permittent with INR of 3.1
4. A possible TIA, yesterday, with right arm weakness and numbness
EKG shows atrial fibrillation with a controlled ventricular response, and a T-wave
inversion consistent with ischemia. This sounds like the same appearing EKG that
has previously been done in January, however The physician does not have the EKG
for a comparison. Patient is on Sotalol and would appear reasonable if patient
would stop the medication. The Patient occasionally had Angina Pectoris about
twice a month relieved by Nitroglycerin. Patient is followed by Doctor____.
